Portfolio Information

Missing forms?  Need extra copies?  Browse below by category to find important templates you will need to complete your portfolio.  If you have questions or need additional help, see Ms. Kirklin in room 8 or your C&C Coach


  • SMART Goals
  • Transcript: copy of most recent transcript available in counseling office
  • Credit check: copy of current credit check
  • 4 year plan 4 year plan completed in 9th; updated yearly if needed 10th 11th 12th
  • Assessments:copies of all available completed test (HSPE, ASVAB, COMPASS, PSAT/SAT/ACT, MAP, CBA, etc.)
  • Evidence of Work: minimum of 3 for each year in attendance at Franklin Pierce Schools, preferably examples of best works that reflect career path focus



  • Volunteer Service: Complete and document a minimum of 30 hours of community service.
  • Extracurricular activities: Certificates, photos, awards, or other evidence to demonstrate involvement in school and community activities.

  • Senior Presentation rubric: Every senior must present his/her portfolio to a panel of community members
  • Distinguished Portfolio checklist: Students who earn "distinguished" status during their portfolio presentations are eligible to apply for the Franklin Pierce Foundation Distinguished Navigation Scholarship. (This checklist covers portfolio content requirements.  See the Senior Presentation rubric for distinguished presentation requirements.)
